Is your content request related to a problem? Please describe. Our consent form is usually two pages, and the first page can be left behind with the participant for some high level project descriptions. However this doesn't include contact information and is usually not written in plain language. As part of an ethical research practice, we should leave behind an information sheet.

Describe the solution you'd like Information sheet template that outlines:

• About the project
• Important Information (around privacy etc.)
• Who to contact for questions
• If you need support (depending on context of research problem)
• Contact for more information on the service
