Cold/Flu: Include registrant name beside submitted form on Admin list

User Story As an administrator I need to be able to search or filter submitted Influenzas Worksite Registration forms by registrant so I can quickly identify and edit submissions

Tasks Steps needed to achieve this feature.

Acceptance Criteria similar functionality to other forms where users can be easily seen, and found in the forms name

This functionality is ready to be tested on TEST. Please go to the Submitted Form tab and see the submitter's name included in the application name. This is also has been added to Draft Forms.
