[Tool]: Update Kitchen Sink form #1734

Build a kitchen sink form to test capacity issue for cluster sizing.

This form needs to be accessible without authentication and have similar components/features like our DJ form to ensure we are able to test the capacity changes and infrastructure as code solution properly
