Simplified disease names to match generic naming from SNOMED-CT

@bradhead will this be the format for compound vaccinations? Feels like if it was CSV format might keep it a bit more standardized for parsing and then it's the responsibility of the web client to display the data logically versus the API to dictate its presentation.
