Collecting survey feedback on Business Registry apps #10210

As a client When I use the BC Registry application To complete a filing I am able to provide feedback on my experience

Here are the questions we would like in the survey for the Business Registry apps (Create UI, Filings UI, Edit UI, Name Request). @jyoti3286 @mstanton1 to confirm when/where to add to PPR and AUTH UIs.

  1. Do you agree that [app name] was easy to use?
  2. Do you agree that the information and instructions provided by [app name] were easy to follow and understand?
  3. If you contacted the [app name] Help Desk, how satisfied are you with the assistance provided?
  4. Overall, how satisfied are you with [app name]?
  5. Adding a Comments text box - MD

@lmcclung on the assets action item to identify when/where to add the survey in PPR can you confirm briefly what is being done in entities? I will try to be consistent, where it makes sense.

@pwei1018 looking for feedback on how frequently and where to add this survey. Right now we have the feedback button always present on Names Application.
Would the survey is activated by this feedback button? Is there a way to only display it at a certain point in a filing (Corporate online does it at the end) or will it always be there?

Notes: The current questions are clearly biased / leading. This is almost certainly leading to overly positive responses. I recommend rewording the questions to remove bias. Since we are talking about rating a different version of the site, we need to consider if we want to change the questions to rate the new version. If we change the questions and have the new ratings apply to the new site only, we introduce a confounding variable, making it difficult to compare ratings of the new site and the old site. We also need to consider if makes sense to continue to collect biased data.
