UI Design - Update Add Existing Business/Name Request + Forgot Password Flow for Corps/Coops/SP/GP

Both Staff Users and Regular Users need the ability to:

1. Add an existing Corp/Coop/GP/SP business manually to a user's My Business Registry table (modal needs updating)
2. Add an existing Name Request manually to a user's My Business Registry table (modal needs updating)
3. Retrieve a forgotten password/passcode (flow is broken for Corps/Coops and non-existent for SP/GP)
4. Optional: update an existing password/passcode

Notes:

• Fix broken "Forgot password" flow and include flow for GP/SPs
• Update modal language and form fields to include SP/GP businesses

@lmcclung - Here is the other design ticket to update the flow for adding a business or name request to a user's My Business Registry table and retrieve a forgotten password/passcode so this works for Corps/Coops/ and SP/GP
