Form 03 PPR

Form 03 PPR requires the following changes. Note, these changes must be pushed to PROD at the same time as Form 03B. When complete please provide to Megan for review and discuss with staff before having them pushed to prod

Changes

• [ ] Remove second bullet in the INSTRUCTIONS reading "There is no filing fee to issue a new code."
• [ ] Remove 'DO NOT MAIL' wording at the bottom of the form. This is currently a manual process and electronic or mail submissions are required.
• [ ] In Section 1 Application Request above the applicant name include a field for "BC Registries Account Name" and "BC Registries Account Number".

The current Form 3 on the BC Registries website does not include the changes noted above: https://www2.gov.bc.ca/assets/gov/employment-business-and-economic-development/business-management/permits-licences-and-registration/registries-forms/form_03_ppr_secured_party_codes_changes_to_prior_registrations.pdf.

I have looked at the draft above and it is not rendering with spaces to enter information. This work is still pending, but given the time lapse, is not a priority. Can you have this as a backlog item for your forms person?

@mstanton1 - is this forms work still outstanding? It maybe be helpful to complete it prior to the wave of CORPS forms. Please let me know if there is any specific work required for the PDF forms designer.

@daniwilliamson this form has not been implemented in PROD. It will need to be implemented at the same time as the form in Zenhub 12559. I can review both drafts with the business and then provide you with an update on the work remaining.

@daniwilliamson I've scratched out my last comment. This form was implemented. However, the business noted clients don't always provide an email in the authorization section so they have requested that the current form be updated to include a phone number field in the Authorization section before this is closed.

This form was done by Gen and she should be able to access it. If the forms designer is using the same tool this is probably a really quick fix. If the forms designer is using a different tool it may be worth seeing if we can pull Gen's time to make this quick adjustment.

Note: once updated this form would need to be uploaded onto the CMS site to replace the existing form (https://www2.gov.bc.ca/gov/content/employment-business/business/managing-a-business/bc-registry-services-personal-property-registry#forms-fees)
