Certify/Agreement checkbox missing 3rd box #24472

Repro Steps:

Go to Test > Platforms > Application
Fill in form and go to last step in the wizard
Scroll to bottom of page

Actual: There are only 2 agreements/certifications

Expected: There must be a 3rd box for:
