Update Email Address and Address on Identity Affidavit

The identity affidavit that is available through the Business Registry at https://test.bcregistry.ca/business/auth/nonbcsc-info/download contains contact information in the header.

BC Online Administration provides support for affidavit's so the BC Online Help Desk has requested we include the BCOL Admin email address in the header instead of listing the BCOL Help email. Currently, BCOL Admin does not have a posted phone number so in order to retain a phone number the BCOL Help number must be used.

While making these changes the E should be added to the mailing address on the affidavit.

Impact: BCOL Help noted that a few clients have completed the affidavit and emailed it to them. Updating the email address would take them out of the process when a client erroneously emails the affidavit in, instead of completing the account creation request in the system.

• [ ] Add Entity or Relationships label to zenhub ticket

  • If the business says it is a priority
  • BA can use their business knowledge and best judgement if it is a priority or not
  • How long the ticket has been open
  • If we are still unsure, reach out to other BAs in the guild
• [ ] When ticket has been created, post the ticket in RocketChat '#Operations Tasks' channel
• [ ] Reply All to IT Ops email (CC BA Inbox) and provide zenhub ticket number opened
• [ ] Dev/BAs to resolve issue
• [ ] DEV: Enter the time you worked on the ticket in the estimate
• [ ] DEV: Tell BA it is ready to review
• [ ] BAs review
• [ ] Add ticket to the current milestone of the code base the ops ticket was relating to (i.e., if it's a Name Request issue, assign to current entities milestone, etc.)
• [ ] Close Zenhub ticket
• [ ] Tell IT Ops to close the ServiceNow Incident

@seeker25 Not quite as this ticket is from September 2021! But we can totally reuse this ticket for the PDF replacement. John asked that I inform him when I go about this work with you so that he can work it into a plan. Right now this lives with @sarahelz19 who is doing final checks on the affidavit. Sarah, interesting case for BCOL address. We had talked since with the business and we had decided to scrap the address entirely, which may actually help clients in not sending it to the registry instead of uploading in the application.
