Overview

This PR includes the following proposed change(s):

Set disabled to true for all non-author users of a form
- Codes will load from 'View state' and show the values that were previously missing
Give Admin roles an edit button for forms that aren't their own
Remove useEffect and set values as useStates
Closes #3562
