Update Name Request Front Page #8715 #457

Issue #: /bcgov/entity#8715

Description of changes:

In the name request page:

  1. put the list item (options) of a dropdown under the select input, instead of on the select input
  2. change the color of an unselected list item to #495057
  3. change the Jurisdiction list item text of 'BC' to 'British Columbia'

In edit name page:

  1. adjust column width
