17152 Conditionally hide Jurisdiction (and Entity Type) menus

Issue #: /bcgov/entity#17152

Description of changes:

app version = 5.0.11
replaced some local methods with direct store actions
fixed responsive layout
added handler for request action change (instead of set + watch)
added dummy input box
added conditions to show Jurisdiction
added conditions to show Entity Type
misc cleanup (indents, whitespace, comments)
deleted redundant typing
deleted some accessors that caused template warnings

Guys - Yui reviewed this and made some comments in the ticket itself, but I'm going to merge this ASAP (so you can rebase and build upon this) and then I'll create another PR for Yui's changes.
