17630 Reset request action on start over #690

Description of changes:

Small bug fix based on Yui's comment. Resets the request action component when the "Start Over" button is pressed.

IMO, app version update is not critical for this one, since we just had a bunch of merges at once and we haven't deployed anywhere yet (Dev, Test, Prod).

Ah just saw this after updating the version, will keep in mind going forward

App version is most useful to determine what features to expect in a deployed UI.
