This change is to resolve the issue with duplicate action codes. It adds a unique constraints to the action_type_action_xref table that prevents inserting records with the same combination of action_code and action_type_code.

[x] After the database container is fully initialized try to run the following SQL statement INSERT INTO case_management.action_type_action_xref( action_type_code, action_code, display_order, active_ind, create_user_id, create_utc_timestamp) VALUES ('COMPASSESS', 'ASSESSHIST', 3, 'Y', CURRENT_USER, CURRENT_TIMESTAMP);
It should return an error that indicates that inserting duplicate actions is no longer possible
