Design - Receive LRS Application

Describe the task Create low-fi designs for the LRS application including staff and SDM review.

Dev needs hi-fidelity to properly estimate on their task.

Acceptance Criteria

[ ] Low-fi designs created for assign application
[ ] Low-fi designs created for staff review
[ ] Low-fi designs created for SDM review
[ ] ??? Do we include SDM Approval and Rejection? Probably need designs for kicking it back to reviewers and the entire flow.

Additional context

? Unable to proceed until questions content of the forms are answered, after #10
Has 3 different user roles(? confirm?) Manager for 'assign application', case worker for 'review application', and SDM for 'SDM Review'.
