Disabling create app features

Changes:

1. Putting banner on the main page.
2. Disabling 'Create App' button.
3. Removing 'Request Account' tab item and route.
4. Removing the text: “*Submit the Request Account form to add a new application”.
5. Putting the banner above the header on My App page.
6. Deleting Document page and removing from navigation bar, route deleted as well.
7. About page, disabling “Request Account” links and "Documentation" links as well, replacing with catalog site url: https://bcgov.github.io/common-service-showcase/onboarding/aps.html
