... which is correct, based on the data in COLIN. (The org name hasn't ever changed so the name effective date is the same as the org registration date.)
The filing code of OTANN translates to "Annual Report", and the effect of the filing was to change the org status from Historical to Active (hence the latest credential). This matches the latest filing in COLIN.
So it looks like there are a couple of issues in OrgBook UI:
the name effective date as displayed is incorrect
the display name for the OTANN filing may be incorrect (we do a mapping for what is actually displayed in the OrgBook, so I don't know offhand how this code is treated).
In OrgBook for this org (https://orgbook.gov.bc.ca/entity/CP0002200) and the info in the credential itself looks ok. The most recent credential contains:
