Create a data dictionary for all PDP data fields and flow #1131

We need to create a data dictionary that we can provide to the data warehouse to assist in building and maintaining the more advanced business intelligence dashboard.

We need to list out all the tables that we are sending to the data warehouse. For each data element, we need to have a description (i.e. what is this used for, what does it capture), data type (integer, text, etc), and data length limits or other restrictions.
