Option for disabling toggle links #212

The plugin works great, but it would be nice if there were an option for hiding 
elements completely instead of replacing them with a clickable toggle (e.g. 
gbarToggle and headerToggle in reader). I want them gone, like really gone, and 
the 10 pixels taken up by each toggle doesn't feel very minimalist. ;-)

The extra 20 pixels of white-space at the top of my reader is actually rather 
distracting, and every pixel counts on this tiny netbook screen.
