Make responder.lua hangup when the call appears to be going nowhere #58

This is in case the call is not making any progress in a long time, either the 
call didn't get through properly or the caller is confused or the call went to 
voicemail, etc.

See survey.lua for what happens when there are more than n prompt replays in a 
row
