documentation: clarify how smart_importer works on transactions and protocol needed for extract method, per seeing issues like https://github.com/beancount/smart_importer/issues/88 and https://github.com/beancount/smart_importer/issues/87 (which were also the issues I encountered when first trying smart_importer)
