Out of sync with gutter icon and notes tool window line numbers or deleted #41

  1. Delete a note in the notes tool window, the gutter icon still exits
  2. Change database connection to empty, the gutter icon still exits
  3. Update to a new database connection, the gutter icon can't be updated
  4. Drag a gutter icon to a new line, but the line number in the URL editor still is wrong
  5. Edit to a new line in the URL editor dialog, but the gutter icon can't update to the new line
  6. If the notes.db file is modified out of plugin, refresh button in Notes tool window doesn't refresh gutter icons
