After installing Toolbox, options are not appearing in menu - not all Toolbox options are available

What steps will reproduce the problem?
1. Anytime I open Toolbox extension window

What is the expected output? What do you see instead? The list of various 
toolbox options does not appear. When opening Toolbox only a few options 
appear. I have attached a screenshot

What version of the product are you using? On what operating system? I just 
downloaded yesterday, so this is the new version I belive
