Add constraint check on dates of an appointment

Add constraint check on start and stop dates of an appointment to prevent an appointment from being created outside of available timeslots

• [ ] Create widget for appointment slot validation
• [ ] On scheduling page in new appointment form use this widget to validate selected time
• [ ] If time is not in correct times lot, show warning message.
