Problems in eMI MCQ component r.e. question numbering #207

When you enter the component, answers begin at Answer 0. This is confusing 
for a user, as the first answer will be 1. This was reported earlier.

In the boxes that pop up for entering feedback, the first number used is 
Feedback 1 (i.e. doesn't correlate with 0).

When you create another question using the 'Add Options' button, it 
appears as 'Answer 4' - starting at 0 it should be 3. 

The problem lies I think with the fact that initially numbering starts 
at 'Answer 0', which should be changed to 'Answer 1'.
