revision 330 stim issues #9

while using olstim in revision 330-
1) found one instance where the stimIndices were being removed while SALPA was 
using them, causing an index out of range error- it looks like ephysacquire 
removes stim indices in a non safe manner- need locks around at least the stim 
indices removal and/or adding
2) found that with SALPA on, and the above locks added, stim artifacts would 
pop up on all channels, but only if the spike filter was on.  Possibly the 
spike filter is decreasing amplitude enough for said artifacts to get through.  
Artifacts are relatively rare (at average stim rate of 10hz on one electrode, 
get maybe 1 ever 2-3 seconds).

This bug should be fixed due to recent SALPA revisions
