Refresh Captcha after Too Fast #322

I'm creating local users for test and I triggered the "too fast" error from the captcha.

At this point, the captcha has changed but the image on the form is not refreshed, so clicking submit again triggers an incorrect error.

clicking off and back on again refreshes the image. obviously not high priority, just noting here.
