Top Menu Bar

[x] “Save” button saves the student's work, but no popup when clicked as it currently does. Button gets "active" style when there are changes to save. becomes "inactive" style when there are no changes to save. Add time stamp to right of save button for most recent save.

[x] Add Question prompt/description in a new bar below the top menu bar

[x] Move alert box for adding description just below the text area to make it more noticeable to students.

Do not change the back arrow to "all problems". Keep as the back arrow button

just a couple things @benetechMatt :

1) For the help button, I would advise against changing from the icon. Icons can be more accessible for students with language difficulties, from students with LD/cognitive disabilities to ELL students, particularly if they're consistent (eg back buttons, help icons). having text on hover ("help") could be useful though.

2) I'm wary of showing "last saved" timestamp because it can add more clutter to the menu bar

3) The math prompt needs to be at least 18pt font or bold 14pt font for that color combo (WCAG accessibility guidelines)
