Set Up Recurring Form

 User story definition

• On the Manage Forms page, add an action to the form's drop down called 'New Recurring Form'.
• Invoking the action should create a copy of the form, linked to old one and take the user to the 'Form Details' page.
• The default title of the new form should be "$oldFormName (Created $dateStamp)" and should be highlighted for text editing by default. \ Should we prevent the recurring form being edited to have exactly the same name as its linked form?
• The Design page should not exist.
• Subject selection should be editable, but subject type should not.

  Additional UI

• The Published/Draft label on the Edit form page should read 'Recurring Form - Draft' or 'Recurring Form - Published' as appropriate.
