Crash when scanning QR code for address book #200

What steps will reproduce the problem?
1. Go to the address book and press the "Scan address" button
2. Scan a QR code containing a bitcoin: URL with no label or amount specified, 
holding the phone in portrait mode (for example the code at 
http://themonetaryfuture.blogspot.co.uk/2013/07/why-i-accepted-executive-directo
r.html)

What is the expected output? What do you see instead?

The "Add address" dialog pops up as expected.  If the screen is now rotated to 
landscape, the app crashes.

What version of Bitcoin Wallet are you using? On what Android version and
what phone? Don't just state 'most recent', this is not enough info.

Bitcoin Wallet 3.13
Android 4.2.2 on Galaxy Nexus

Please provide any additional information below.

If the code is scanned with the phone held in landscape mode, the dialog box 
pops up and the app does not crash when rotated between landscape and portrait 
mode - however, the buttons at the top of the screen are duplicated as in issue 
#199.

I'm having this same issue on Galaxy Nexus, CM 10.2, Oct 6th Nightly and 
Bitcoin-Wallet 3.21.  The app only crashes in Portrait mode.  In Landscape 
mode, this doesn't happen.  However, I can only have the phone in a single 
landscape mode, not reversed 180 degrees (so the charging pogo pins face down).

I believe the crash is now fixed in recent versions.
