Various performance enhancements #17

  1. add check for min number of obs per class for dichotomous outcomes
  2. update how iip is computed to match wagh et al (2016)
  3. update how iip is printed in report
  4. update dockerfile to pull vimp 2.0.1 from github
  5. add new environment variable "nfolds" to specify number of folds (defaults to 5)
  6. add necessary r scripts to dockerfile
  7. add log file to print i/o stream to (rather than console)
  8. add prefix "nab_" to column names in analysis data frame (removes potential for unexpected 'X' prefixes if the nAb has a numeric start, e.g., 10-1074)
  9. add check to change backslashes in nAb name (e.g., VRC01/PGDM1400-10E8v4) to dashes
  10. update README
  11. add example_runs directory and populate with VRC01 run
