Port to Silverstripe 4 #28

Initial port to work with Silverstripe 4.

Includes:

Name space changes (I hope this name space is ok? Otherwise I can change it easily enough :) )
Template structural changes to work with new name spacing
Readme updates

@benmanu any chance you could merge this? If the namespace isn't right, or it no longer works, I can fix it up. Its a really useful module, so would be nice to be able to easily use it with 4 :)

Hey @adrexia thanks for following up on this, I've been a bit slack with this repo as of late! Will merge this in and give it a new tag
