Transfer patient to FP when they exit PNC program

Task

  1. Transfer HIV -ve patients to FP program once they exit PNC program.
  2. For HIV +ve patients, they'll stay in the PMTCT care for 1 year to monitor and prevent mother to child transmission then thereafter, go back to CCC clinic
