-Add radiology report in OPD & MCH clinical history for a patient -Allow the radiology result list to be an expandable list to reveal exact test results

This has some pending task:

The summary in patient history is not displayed correctly as we don't have details like film used.
The system allows double clicking on the record which in turn creates a duplicate record of what is being viewed.
