Not all groups are loaded #11

What steps will reproduce the problem?
1. Start gCalBirthdays 2.08 / 3.03
2. Count Your contact groups

What is the expected output? What do you see instead?
All 66 groups should be displayed.
Only 26 groups will be displayed.

Further:
Maybe the list of calendars is also affected.

Paging for groups and calendars is implemented.
