use regression for plot_cohorts, fit only one model #12

this unfortunately breaks the plot_timeseries model

the direction of this is getting fairly confusing at this point. it's not clear to me if shoehorning everything into regression makes sense. the kaplan-meier interface is kind of messed up, for instance.
