Fixing mapper 4 for some games like Star Wars Episode 4 and Teenage Mutant Ninja Turtles 3.

I also included https://github.com/bfirsh/jsnes/pull/425, https://github.com/bfirsh/jsnes/pull/438 and https://github.com/bfirsh/jsnes/pull/441. Lastly, I'm still checking why sometimes the Star Wars game doesn't load the first level.
