Update readme and migration guide #27

This just rebases the work from #24 on the changes that recently were merged with #26

I didn't review any of this docs, it's all @cah-brian-gantzler 's work 😂 but I think we should just merge it sooner rather than later

I assume this was just updating the readme and moving a couple of the files

The only thing I had to fix was the top-level readme because they style of headings changed to be closer to "regular" Markdown 👍
