Presentational changes to be back-ported from 4.x

The 4.5 branch doesn't seem to have the default comment timestamp fix applied 
to it. This patch brings it into line with the 4.x branch.

On Stef's advice [1], I'd like to change this issue to a catch-all for 
cosmetic/presentational changes that slipped through the `4.x` and `4.5` 
changes. I'll diff /textpattern/setup/txpsql.php between the two branches and 
present the findings with patch files.

I would be grateful if a project owner could edit the issue title to a more 
appropriate title, please - assuming that having a single issue for cosmetic 
stuff is preferred to a bunch of new issues for what might be a trivial patch.

This patch covers a handful of hardcoded dates (switched to `now()` instead) 
and category title capitalisation. Nothing new, just migrating some changes 
made to `4.x` into `4.5`.
