After running "percival finish", unconverted refs remain in the code #26

There were six occurrences of the above in Miller's book.

They all occurred within ol and blockquote tags, apparently.

The book Miller was working on was LeadRevisited

Marvin also experienced this, to the tune of 162 instances on NewStudy, 9781535974431

He said he saw them in a mix of places, paragraphs, tables, not the same as in Miller's book.

This may or may not be related to #25

Investigate percival not finishing properly
