Urgent

Redirect at the repo root to cater for traffic from npmjs.com needing V3 docs but arriving at v4-development default branch.

Some more improvements to docs.

Copy for docs/how-it-works.md.
Fixes to docs/troubleshooting.md URLs per #182.
Minor tweaks to copy on Readme.md.

Misc

svg code-formatted, no changes.
minor fix to sourcemap-to-string that has been broken at some point by node-convict update.
