Updating LCF documentation for release v1.3.0 #303

Editorial tidy-up of LCF documentation for release of Code Lists Issue 5, Implementation Profiles Issue 3 and LCF v1.3.0.

NOTE: Release dates have NOT yet been added.

Code Lists:

most of the changes replace references to LCF version numbers (e.g. Added in v1.2.0) with references to Issue numbers (e.g. Added in Issue 4). Rationale: Code Lists are identified by Issue number, not by LCF version number.

General:

updated placeholder version number from 'vx.x.0' to 'v1.3.0';
updated links to BIC license;
minor editorial tidying to improve consistency.
