User Case-studies Form #3

See 'case-studies' tab in header. We need a form so that when a user clicks 'tell your story', they can complete details on a case study that they have had success with from IFC and the ecosystem to share on the page.

Once complete, please copy new form address to project path (case-studies/index.html).

Buildingsmart will require control of this so if you create it , please take steps to update docs site maintainer with details and so we can publically thank you for your work.

Decision to remove based on duplication of content in buildingsmart.org site instead.
