Add docs for flashing firmware and bootloader #491
My SD card slot wasn't working so I had to find out how to flash via USB. In the process I found out how to flash the bootloader, because I thought you were using the vanilla Smoothieware bootloader which uses pin 2.12 to flash (so I loaded a modified version onto it). I think this is useful information for others, though feel free to suggest a better place than the top level README (while the pinouts might be 1.4 specific, the method should work for both boards).
