"Citation continues on next/prev page" behavior #45

Currently it may not be clear to a user that a citation spans pages, and thus they may not fully review the citation in context.

Some ideas:

Require user to view all the pages of a citation
- Perhaps one sidebar entry per page, e.g. two "approves"
Move (or add) "citation continues on next/prev page" near outline
