A patient-reported outcome measure that assesses the personal impact of HS
Consists of 12 items across 5 domains: symptoms, daily activities, emotional well-being, treatment, and relationships
Each item is scored on a 5-point scale (0-4), with higher scores indicating greater impact
The total score is calculated by summing the scores for all items and ranges from 0 to 48
Higher total scores indicate a greater personal impact of HS

Output will be a numerical value.

If the questions are identical to those in #6, please use the same parameter names. If they are not word-for-word identical, please don't use the same parameter names. (Clinical validation of tests requires exact consistency. We don't want to accidentally mix them.)
