For example: when the porch light is on and home-assistant restarts, they turn off.

This is likely related to the way home assistant works with zwave2mqtt.

Perhaps there is an 'initial state' that somehow needs to be set
Perhaps there needs to be something set to preserve the last setting (I think this is something you can configure in zwave2mqtt)
