Fix Contributing section #463

Presently it is a self-referential loop between wiki and README.md. Find original text, and decide on duplicating between the two.

Add discussion of wiki vs code contributions. Add list of CREDITS.

Cleaned up README.md and wiki page, and both point to https://github.com/binkley/modern-java-practices/blob/master/CONTRIBUTING.md rather than each other.
