Create organism sheet has "description" field twice #80

In the sheet that comes up for organism it would be good to have a field for genotype instead of/in addition to description. There also probably shouldn't be a sequence field for an organism. Then again maybe just leave it because people could maybe want to record the sequence of some integrated DNA cassette.

I removed description, added genotype and left sequence in place.
