add information about the system and where it was developed #102

In the box on the bottom of the main pages (top-level index and per-individual index pages) we should add the following text:

The My Microbes system was developed in the Caporaso Lab at Northern Arizona University and the Knight Lab at the University of Colorado, in collaboration with the Fierer Lab at the University of Colorado and the Dunn Lab at North Carolina State. Questions about the My Microbes system can be directed to student.microbiome@gmail.com.

We have that link at the top already, but agree that it'd be good to add here as well (linking My Microbes to the repo would be perfect).
